Accompanying Your Child

Pediatric Associates of Conn, PC understands that you may not always be available to bring your child to a scheduled appointment, specially to pediatric well visits and sports school physicals. We ask that you complete a portion of our demographic sheet naming and giving permission to the person or persons that may have occasion to bring your child into the office. Pediatric Associates of Conn, PC may request identification of the person that accompanies your child to confirm they are indeed the individual listed on the Consent for Treatment form.

Please be available by phone so our provider can contact you for permission and details of what you need to be aware of.

Patients 18 years of age and older may come to appointments unaccompanied to receive lactation counseling or any other of our CT services.
