UNFORNTUNATELY, OUR OFFICE DOES NOT HAVE DETAILS FOR YOUR SPECIFIC INSURANCE COVERAGE. PLEASE CONTACT YOUR INSURANCE COMPANY DIRECTLY WITH ANY QUESTIONS AT THE NUMBER LISTED ON THE BACK OF YOUR INSURANCE CARD.

YOU WILL NEED TO CALL THE INSURANCE COMPANY AND ANSWER THEIR QUESTIONS OVER THE PHONE OR BY MAIL. IF THIS IS NOT UPDATED THEY WILL NOT PAY YOUR CLAIMS AND IT WILL BE PATIENT BALANCE.

IF YOU UNABLE TO PAY YOUR BILL IN FULL, MONTHLY PAYMENT PLANS ARE AVAILABLE AND CAN BE ARRANGED BY CONTACTING OUR BILLING OFFICE AT (203) 755-2999 EXT 126, 127, 129, 147 AND 155. WE ALSO ACCEPT PAYMENT OVER THE PHONE BY CREDIT CARD.
STATEMENTS ARE SENT OUT MONTHLY AND IDICATE BOTH THE PATIENT AND INSURANCE DUE BALANCE. YOUR STATEMENT WILL REFLECT ONLY TRANSACTIONS POSTED WITHIN THE MONTH OF YOUR STATEMENT.
